New TODCM features in Aug/Sept

1. Teacher evaluation - Code is already in the version 1.1.0.3, version 1.1.0.4 will add generic template.
2. Student and teacher surveys - This is an integrated part of teacher evaluation. Code is already in the version 1.1.0.3, version 1.1.0.4 will add generic template and make LDAP login optional for students. The code in 1.1.0.3 requires students to login thru LDAP.
3. Login security with CAPTCHA - The current 1.1.0.3 behavior is to lock out an account for 15 minutes after 5 failed attempts. The new CAPTCHA feature will not lockout the account but the user has to enter a string from a generated CAPTCHA image. The account lockout and CAPTCHA will have 2 admin configuration options to allow you to toggle between the 2.
